Other Application Materials

In addition to information submitted online through the Online Application process, the other materials listed below are required. Mail these materials to the address below:

  1. 1. Official transcripts from each relevant institution attended

    Request that one official transcript from each relevant college or university attended be sent to the Information School. Relevant transcripts include transcripts which account for the last 60 semester (or 90 quarter) credits of graded work, transcripts from institutions from which you received (or expect to receive) a degree, and transcripts for any other academic work that you would like the Admissions Committee to review. Each transcript should be sent in an envelope sealed by the institution; photocopies of transcripts or transcripts not in sealed envelopes will not be considered official.

  2. 2. GRE and GMAT scores

    Graduate Record Examination (GRE) or Graduate Management Admissions Test (GMAT) score reports should be sent directly to the Information School. The institutional code for the GRE is 4854; you do not need a department code. The GMAT report code is 459-RN-51.

    Note: We accept GRE and GMAT scores of any age from the following official sources:

    • Educational Testing Service (ETS - GRE reports), Graduate Management Admissions Council (for GMAT reports), (ETS will retain GRE score records for 5 years.)
    • A sealed, paper original, official GRE/GMAT score report mailed directly to our office
    • Official transcript that lists your GRE/GMAT scores: Contact school directly to determine if they report GRE scores on transcripts
    • GRE/GMAT scores reported to a different school: Request that the school send a list of your GRE/GMAT scores (with percentages) on school letterhead, signed by a school official, in a sealed envelope.
    • Copy of your official score report submitted by a previous school with a note (on letterhead) verifying that the copy was made from an original document: We can accept GRE/GMAT scores from other schools, but require that this information be sent in a sealed envelope and include verification or documentation on school letterhead and be signed by a school official. We cannot accept email attachments or faxes.
    • If you cannot obtain an official score report for your previous GRE (or GMAT) scores, you will need to take the test again.

    Applicants with an earned doctoral level degree from an accredited institution in the United States (PhD, MD, JD, etc.) are not required to submit GRE or GMAT scores.
  3. 3. English language proficiency requirements

    Proficiency in English is required for graduate study at the University of Washington. Therefore, every applicant whose native language is not English must demonstrate English language proficiency. Proficiency can be demonstrated in one of the following ways:

    • A bachelor’s or doctoral degree (but not a master’s degree) from a regionally accredited institution in the United States or from an institution in Australia, the Bahamas, Canada, Ireland, Jamaica, New Zealand, Singapore, Trinidad and Tobago, or the United Kingdom.
    • Official documentation from the institution verifying that the applicant’s undergraduate degree is from an institution (in a country other than those listed in #1 above) where all instruction is in English. The undergraduate degree must be equivalent to a US bachelor’s degree. Documentation must be presented to the Office of Graduate Admissions.
    • TOEFL (TOEFLiBT, TOEFLC or TOEFL), IELTS or MLT official test scores

    Applicants who are non-native English speakers must submit documentation, in one of the following forms listed above, for their application to be considered complete. Documentation must be submitted to both the Information School and the Graduate Admissions Office. (Use Graduate Admissions address provided below in Foreign transcripts section if you are mailing the documentation. Test scores can often be reported electronically.)

     

  4. 4. Foreign transcripts

In addition to submitting official transcripts to the Information School, applicants with foreign transcripts must also submit them to the UW Graduate Admissions Office. Please reference Required Materials for International Applicants section of the UW Grad School website (http://www.grad.washington.edu/admissions/intl-materials.shtml) Although the instructions given are for international applicants, if you are a US citizen or permanent resident with foreign transcripts, you will need to follow the same procedures for submitting your transcripts, (ie, one copy to the Information School and one copy to UW Office of Graduate Admissions).

NOTE: The Information School has no access to check if materials were received in the Office of Graduate Admissions. Please use the contact information at the bottom of the "additional required materials" page above to contact Graduate Admissions.

5. Financial Ability Form - International applicants

The U.S. Department of Homeland Security-(DHS), and U.S. Immigration and Customs Enforcement-(ICE), mandate all schools, including the University of Washington verify certification of adequate financial support from each international applicant without resorting to unauthorized employment. Therefore, all international applicants, regardless of visa status, must submit the Financial Ability form and appropriate documentation. The Financial Ability form becomes available through a link in the list of task items after the MyGrad application is submitted. (You can complete the UW Statement of Financial Ability form and send with original financial documentation after the application deadline.  However, this could result in the delay of a formal offer of admission.  It is highly recommended you complete the financial ability requirements as soon as possible after submitting the MyGrad application.)
