• Internship Application

Complete this application and bring with you to your internship appointment with the MSIM Academic Advisor. The application will give your advisor information about your areas of interest and goals for the internship that would help facilitate with placement.

• Course Checklist

Complete this document and bring with you to your internship appointment with the MSIM Academic Advisor. This indicates your completion of all required core courses prior to starting your internship.

• Internship Confirmation

Use this updated confirmation form for internships beginning in summer 2010. After you have secured an internship position, please complete and digitally sign this confirmation form; then route it to your supervisor for a digital signature. Route completed form in to Kath Gold at kgold@washington.edu (MGH 470) to receive an add code for IMT590 registration.Please note: you must have a supervisor signature to receive an add code.

• Learning Objectives Agreement (LOA)

At the start of the internship, you and your supervisor will draft a learning objectives agreement outlining goals to be accomplished, tasks to meet the objectives, and evaluation methods to assess progress. This agreement is due to the IMT590 faculty chair by the first week of the quarter.

• Mid-Internship Report

At mid-quarter, your supervisor will complete an evaluation on your progress and adjust the LOA if necessary. This completed and digitally signed report is then sent to the IMT590 faculty chair for review.

• Outline for Student’s Final Narrative Report

At the end of the internship, you will complete a final report on your experience with the internship and the host site. You may include your evaluation of the host site as well as give feedback on the internship program. This is sent to the IMT590 faculty chair for review prior to your exit interview.

• Supervisor’s Final Evaluation

At the end of the internship, your supervisor will complete a final evaluation of your work, determining the completion of your stated objectives in the LOA. The completed and digitally signed evaluation is sent to the IMT590 faculty chair before the end of the quarter to allow for review and assignment of credit for the IMT590 course.
