MLIS: Other Application Materials
In addition to information and materials submitted online through the MyGrad application additional materials must be sent to the Information School:
MLIS Admissions Committee
Information School
370 Mary Gates Hall, Box 352840
University of Washington
Seattle, WA 98195-2840
1. Official transcripts from each relevant institution attended
Request that one official transcript from each relevant college/university attended be sent to the Information School. Relevant transcripts include transcripts which account for the last 60 semester (or 90 quarter) credits of graded work, transcripts from institutions from which you received (or expect to receive) a degree (excluding associate's degrees), and transcripts for any other academic work that you would like the Admissions Committee to review. Each transcript should be sent in an envelope sealed by the institution; photocopies of transcripts or unsealed/opened transcripts will not be considered official and will not be accepted.
If you are applying to the Law Librarianship program, you must submit transcripts from the institution where you earned your JD degree.
2. Official test scores
GRE
Request that your official GRE scores be electronically reported to the UW using code 4854. We can also accept an original GRE score report, sealed in the original ETS envelope. If your scores were previously reported to another school, we can accept a list of your GRE scores with the date you took the test, printed on school letterhead and signed by a school official. We can also accept a copy of your official GRE score report sent from a school you previously attended. However, in addition to the copy of the official GRE score report, the school needs to provide a letter verifying that the scores are accurate and that they made the copy. All documentation of this nature needs to arrive in our office in a sealed envelope, unopened by the applicant.
(Applicants with a doctorate or professional degree (PhD, JD, MD, PharmD, etc) are not required to submit GRE scores.)
If you have previously taken the GRE, but no longer have access to an official score report and no other schools have a record of your official scores, you will be required to retake the test in order to submit official scores with your application.
LSAT
Law MLIS applicants are required to submit an official copy of their LSAT score, in lieu of GRE scores. The LSAC will not send an official LSAT score sheet to our office as the Information School is not a law school. However, we will accept a note from the law school you attended that provides your LSAT score or a copy of your LSAT score sheet made by your law school with a note that verifies the authenticity of the copy. In either case, documentation provided by your law school must be submitted on school letterhead and submitted to our office in an official, sealed school envelope.
3. English language proficiency requirement (non-native English speakers)
Proficiency in English is required for graduate study at the University of Washington. Therefore, every applicant whose native language is not English must demonstrate English language proficiency. Proficiency can be demonstrated in one of the following ways:
- A bachelor’s or doctoral degree (but not a master’s degree) from a regionally accredited institution in the United States or from an institution in Australia, the Bahamas, Canada, Ireland, Jamaica, New Zealand, Singapore, Trinidad and Tobago, or the United Kingdom.
- Official documentation from the institution verifying that the applicant’s undergraduate degree is from an institution (in a country other than those listed in #1 above) where all instruction is in English. The undergraduate degree must be equivalent to a US bachelor’s degree. Documentation must be presented to the Office of Graduate Admissions.
- TOEFL (TOEFLiBT, TOEFLC or TOEFL), IELTS or MLT official test scores
For more information regarding the UW English language requirements, see: http://www.grad.washington.edu/policies/memoranda/memo08.shtml
Applicants who are non-native English speakers must submit documentation, in one of the following forms listed above, for their application to be considered complete. Documentation must be submitted to both the Information School and the Graduate Admissions Office. (Use Graduate Admissions address provided below in Foreign transcripts section if you are mailing the documentation. Test scores can often be reported electronically.)
4. Foreign transcripts
In addition to submitting official transcripts to the Information School, applicants with foreign transcripts must also submit them to the UW Graduate Admissions Office. Please reference Required Materials for International Applicants section of the UW Grad School website (http://www.grad.washington.edu/admissions/intl-materials.shtml) Although the instructions given are for international applicants, if you are a US citizen or permanent resident with foreign transcripts, you will need to follow the same procedures for submitting your transcripts, (ie, one copy to the Information School and one copy to UW Office of Graduate Admissions).

5. Financial ability form - International applicants
The U.S. Department of Homeland Security-(DHS), and U.S. Immigration and Customs Enforcement-(ICE), mandate all schools, including the University of Washington verify certification of adequate financial support from each international applicant without resorting to unauthorized employment. Therefore, all international applicants, regardless of visa status, must submit the Financial Ability form and appropriate documentation. The Financial Ability form becomes available through a link in the list of task items after the MyGrad application is submitted. (You can complete the UW Statement of Financial Ability form and send with original financial documentation after the application deadline. However, this could result in the delay of a formal offer of admission. It is highly recommended you complete the financial ability requirements as soon as possible after submitting the MyGrad application.)
Please review the International Applicant section of the Grad School Admissions site for more information: http://www.grad.washington.edu/admissions/intl-materials.shtml.
If you have questions about the application process, please contact the Information School’s Office of Student and Academic Services (OSAS) by e-mail at either mlis@washington.edu or onlinemlis@washington.edu, or by phone at 206-543-1794.
