Directed Fieldwork Forms
Please refer to the Directed Fieldwork Student Checklist for more information on setting up and completing your Directed Fieldwork. DFW students work with Marie Potter, MLIS Academic Advisor (206-616-2544, mardup@uw.edu), to set up the fieldwork experience, and with the DFW Faculty Coordinator (instructor of LIS 590) to complete the fieldwork experience.
Directed Fieldwork Forms
1) First, review the PowerPoint as well as the online materials, then turn in the following items to the MLIS Academic Advisor:

3) Confirm your host site then submit the following form to the MLIS Academic Advisor:
- DFW Confirmation and Contact Information Form - must be signed by the supervisor, or must have your supervisor's email verification/approval submitted along with the form
- This form is due no later than four weeks before the start of the quarter in which you will be doing your DFW.
- After you turn in the DFW Confirmation and Contact Information Form you will receive an add code that you will use in registering for LIS 590.
4) During your Directed Fieldwork quarter the following items will need to be submitted by the student and the DFW host site supervisor/mentor. Please submit these items to the DFW Faculty Coordinator after the DFW begins; note tht the DFW Faculty Coordinator will contact you at the beginning of the DFW quarter:

5) After you have submitted your DFW Student Narrative, participate in a DFW exit interview with the DFW Faculty Coordinator.
Law MLIS Directed Fieldwork
1) Law MLIS students work directly with Penny Hazelton, Director of the Law MLIS Program, to set up Directed Fieldwork.
2) Before summer quarter email the MLIS Academic Advisor at mardup@uw.edu to receive your DFW add code.
3) At the end of your Directed Fieldwork quarter the following items will need to be submitted by the student and DFW host site superivisor/mentor. Please submit these items to Penny Hazelton:
