Applying for Graduation

All MLIS students (Residential, Online, and Law) are responsible for preparing and submitting the following items in order to graduate from the University of Washington and The Information School:

  1. Program of Study Form - submit to the iSchool
  2. Master's Degree Request - submit via the Graduate School
  3. Completion of a Culminating Experience

1. Program of Study Form

  • Your Program of Study Form is due on Friday of the 1st week of the quarter in which you intend to graduate.  We recommend submission of your form at least a few weeks prior to this so that Student Services may conduct a prelimiary audit.  The form should be submitted via Catalyst Collect It.  Fill out the form, save the file to your computer, and then upload it to the Collect It site.  You will have until Friday at midnight to submit your form.

    Winter 2010 graduates: due January 8, 2010
    Spring 2010 graduates: due April 2, 2010
    Summer 2010 graduates: due June 25, 2010
    Autumn 2010 graduates: due October 1, 2010


    Please use the appropriate form: MLIS (Residential & Online) or Law MLIS.  Note that if you have a filled out Program of Study form already saved to your computer, you may submit that one via Collect It.  Or you can fill out the identical form available at the Collect It site.
  • Student Services uses the Program of Study Form to perform a degree audit of your record and verify that you have completed or will complete all necessary degree requirements by the end of the quarter in which you plan to graduate. 
  • The Program of Study Form should be filled out with a quarter-by-quarter list of all the courses you plan on using towards your MLIS degree requirements.  You can use your transcript as a guide to filling this out.

2. Master's Degree Request

  • You must submit a master’s degree request online to the Graduate School during the quarter in which you intend to graduate.

3. Completion of a Culminating Experience

Once you have reached 30 credits in the MLIS program you are welcome to begin your Culminating Experience project.  This project must be completed by the end of your final quarter in the MLIS degree program.  Again, you may complete it sooner, but no later than the last day of the final quarter in which you intend to graduate.
 

Additional Information for Graduating Students

  • Washington State Librarian Certification is administered by the Washington State Library in Olympia and is granted to individuals who have earned a Master's degree from an ALA-accredited program in library and information science. The certification application is available online and you must pay an application fee ($20 USD).

  • UW Diploma: You will receive your diploma three to four months after the quarter in which you graduated. Diplomas will be mailed to the address you designated on the Master's Degree Request form —unless you requested to pick up your diploma in person at the Registrar's office. Please notify the Registrar's office immediately if your mailing address changes after you submited your request. If you wish to order additional or replacement diplomas, download the Diploma Replacement Form or contact the Office of Graduations and Academic Records at (206) 543-1803. The diploma replacement fee is $20.00 (USD).

  • Computing Services Information:  As a UW student, you have been eligible for a suite of computing services including e-mail, Web pages, Dante access, file storage, computer lab access, dialup modems, and various other services.

    When do my computing services end? 
    After your last class ends, your student computing services will continue for one full quarter plus about 10 days. Around that time, watch for an expiration notice by e-mail, telling you exactly when your student computing services will end.

    What computing services will continue automatically?  As a UW alumni, the following services will continue automatically: your UW NetID login and password, and your UW e-mail address for use as an e-mail forwarding address. Log in and follow the directions for forwarding your UW e-mail. Note: These services are the only ones that you will keep automatically.

    What computing services will continue if I opt-in?  You are eligible to sign up for continued computing services through MyUW.net, the alumni computing services provider. Note: You must sign up to opt in. Because you will keep your student services for free for a full quarter, it is recommended that you do not sign up for MyUW.net until you receive your expiration notice. At that time you should opt to move all e-mail services (and file and Web page services, if you sign up for them) to MyUW.net, unless you are a UW employee or plan to use a non-UW service provider.
